  Fee Schedule:
  
  • $300 on or before 9/23
  • $360 between 9/24-9/27
  • $450 after 9/28 if entries are still open

  This form may be used if necessary for individuals to send in their own entry information.
  Please note: all relay team members must have their completed entry form, ASA registration (if not already registered for 2011)
  and appropriate fees received by the Meet Director on or before 9/23/11 or additional team fees will apply.
